For flight always have FULL throttle and when you have 15% of fuel in the first stage, separate the side boosters, and separate inmediatly the main booster, before that, gain control in the Polyus, and burn until you are in orbit.

The Polyus had to be mounted backwards due to complicated reasons and is supposed to flip around after separation. But in the actual launch, it did a 360. It is considered one of the dumbest mistakes in spaceflight history by Scott Manley.
Scott Manley in The Dumbest Mistakes In Space Exploration:The spacecraft began performing its 180 degree flip, fired up its engines, and then continues to rotate all the way around 360 degrees. So the engine started slowing the payload down until it fell and was destroyed.
