Auto Credit Based on Pedro's Live representation of control surface angle
What you can see in the image is the craft being still instead of using the RCS to look retrograde.
This happened to me in other crafts in 0.6.9.1 (can't link to them if necessary).
The problem is that the craft has a wrong reference point. If I lock direction and move the nav sphere around the craft rotates but ends up in a wrong "parking" position. The more maneuvers I make the more error is added to the direction of the craft.
